JHA / JSA (Job Hazard Analysis)
A task broken into steps, with the hazards of each step identified and a control assigned to each hazard. It is the document a permit-to-work and a method statement are both built on.
Done properly it is a genuinely useful exercise, because breaking a task into steps forces people to notice the hazard in the step nobody thinks about — the setting up, the moving between, the packing away.
Done as a form-filling exercise it becomes a document that lists "slips, trips and falls" for every task on the project, and it is worse than nothing, because it creates a record asserting that the hazards were considered.
