What is the difference between leading and lagging safety indicators?
A lagging indicator counts harm that has already happened — incidents, lost-time injury rates, fatalities. A leading indicator measures the system meant to prevent it — audit scores, observations raised, corrective-action closure times, training currency. The distinction matters for one reason: a lagging indicator can only ever tell you that you already failed.
The asymmetry
A perfect lagging record — zero incidents this quarter — is compatible with a site that is one unlucky day away from a fatality. Nothing in the number tells you which of those two situations you are in. That is not a flaw in how the number is collected; it is what the number is.
Leading indicators are an attempt to measure the machinery instead of the wreckage. Are inspections actually happening? Are findings closing, and how fast? Is the training current? Does the system find hazards before they find people?
Audits lead; incidents lag
This is the cleanest example of the pair. An HSE audit tests whether the safety management system works — it is a leading indicator. An incident report records what happened when it didn’t — it is a lagging one. They belong on the same record precisely because neither is interpretable alone.
Time-to-close on corrective actions deserves particular attention: it is a leading indicator hiding inside an administrative metric. A site whose closure times are stretching is a site whose safety system is losing, and it will show up in the lagging numbers months later.
The trap
Leading indicators are easier to game than lagging ones, because they measure activity rather than outcome. Nobody can fake a zero-fatality quarter, but anyone can fake a high audit score.
So a leading indicator is only worth what its integrity is worth. Which makes audit integrity itself the indicator that sits behind all the others — the one that determines whether any of them means anything.
