First-Time Pass Rate
The share of inspections passed on the first attempt. It measures whether work is being done right before it is offered — rather than offered, rejected, corrected and offered again.
It is one of the few quality metrics that is hard to game, because the person recording the result is the person who rejected the work. That independence is what gives it credibility.
A falling first-time pass rate is also a leading indicator rather than a lagging one: it moves before the rework cost does, and long before the defects list does. Which makes it worth watching weekly rather than reporting monthly.
